Vaccine Information

Please read all information below prior to scheduling an appointment.

IMPORTANT NOTICE: PLEASE READ

Effective 5/11/23, the COVID federal Public Health Emergency will be discontinued. During the declared emergency all individuals were able to receive the COVID-19 vaccine at no cost to them. Starting on 5/11/23, the Iredell County Health Department will only be able to bill the following insurances for any dose of the COVID-19 vaccine:

  • Blue Cross/Blue Shield
  • Cigna
  • Medicaid
  • Medicare 

Starting on 5/11/23, If you don’t have one of the insurances listed above and want to receive a dose of the COVID-19 vaccine at the Iredell County Health Department, you will be required to pay the current cost for the vaccine. Sorry for any inconvenience.

The COVID-19 vaccine is offered at many other locations in the community. Options for finding COVID-19 vaccine locations near you:

Before you come to your COVID-19 Vaccine appointment, please:

  1. Review the Following Documents:

No longer providing Moderna vaccine

  1. Complete registration form: English Form, Spanish Form
  2. You can receive the Pfizer vaccine if you are 6 months of age and older, parental consent is needed if your are under the age of 18. If under 18, a parent/guardian has to be present to sign a consent form prior to vaccination. You must be 18 years of age or older to receive the Moderna Vaccine.  If you do not meet age requirements and schedule an appointment, your appointment will be cancelled.
  3. Certain population are eligible to receive a booster dose. Please see below for more information.
  4. Do not arrive more than 15 minutes early to your appointment time.
  5. If you have insurance, please provide that information. You will not be billed for any cost of the vaccine if insurance does not pay.

Second Dose Appointment Scheduling:

You will receive a 'Second Dose Appointment Information Card' during your first dose appointment informing you of when your second dose appointment will be.  If you received a 'Second Dose Appointment Information Card', you DO NOT need to make an appointment for your second dose.  If you did not receive a 'Second Dose Appointment Information Card'  or have questions about your second dose appointment, please call 704-878-5300 and press 1.

You MUST receive the same brand of vaccine for both your first and second dose.  

  • Moderna second dose can be administered as early as 28 days after the first dose.
  • Pfizer second dose can be administered as early as 21 days after the first dose.

Booster Dose

Call our office today at 704-878-5300 to schedule an appointment to receive your Pfizer or Moderna booster dose. 


Pfizer: You can get a booster if it has been at least 5 months since your second Pfizer shot.

Moderna: If you were vaccinated more than 5 months ago with the Moderna COVID-19 shot.   


The Moderna booster is a smaller dose than what is given in the first two shots. Be sure to let your provider know you want the booster. 

To make an appointment to receive your Pfizer COVID-19 Booster vaccine, please call 704-878-5300 and press 4. Please bring your COVID-19 vaccine card with you to your appointment to verify you have received your second dose of Pfizer COVID-19 vaccine at least 6 months prior. If you have any questions, please call the Iredell County Health Department COVID-19 Vaccination Hotline at 704-878-5300 and then press 1.

Johnson & Johnson's Janssen

You are eligible for a booster if you are:
18 years or older

When to get a booster:
 At least 2 months after your shot

Any of the COVID-19 vaccines authorized in the United States

Proof Of Vaccination

After you are vaccinated, you are provided with a COVID-19 Vaccination Card.  Please keep this card for your records. 

 If you have misplaced your COVID-19 Vaccination Card and need proof of vaccination, you are able to retrieve this information from the NC DHHS COVID-19 Vaccine Portal.  Please click here to access the NC DHHS COVID-19 Vaccine Portal.  Instructions for accessing your proof of vaccination can be found here.

If you received your COVID-19 vaccine from the Iredell County Health Department and are unable to access your records through the NC DHHS COVID-19 Vaccine Portal, please call us at 704-878-5300 and press 2.  If you did not receive your COVID-19 vaccine from the Iredell County Health Department, please reach out to the provider who administered your COVID-19 vaccine or utilize the NC DHHS COVID-19 Vaccine Portal.  

If you received your COVID-19 vaccine from the Iredell County Health Department and have trouble accessing your proof of vaccination through the NC DHHS COVID-19 Vaccine Portal, please complete the form below and send in to us via mail, fax, or hand-delivery.  Electronic forms will not be accepted.  A proof of ID will be required.  Please see form for further instructions

FAQs

  • An appointment is required. If an individual shows up more than 15 minutes prior to their appointment, they will be asked to return closer to their appointment time.
  • If you schedule an appointment but do not meet pre-vaccination screening requirements, you will not be eligible to receive the vaccine.
  • You must be 5 years of age or older to receive the Pfizer Vaccine (parental consent is needed if you are under the age of 18) and 18 years of age or older to receive the Moderna Vaccine.
  • Proof of residency is not required, but please consider attending a vaccine clinic in your community.
  • Immigration status is not shared with anyone; however name and demographics are submitted to the North Carolina Department of Health and Human Services (NC DHHS) COVID Vaccine Management System (CVMS).
  • Be mindful of the brand of first-dose vaccine you receive.  You MUST receive the same brand of first and second dose of vaccine. EXAMPLE: If you receive Moderna for your first dose you CANNOT receive Pfizer for your second dose.

COVID-19 Vaccine Emergency Use Authorization and Fact Sheets

For each COVID-19 vaccine authorized under an Emergency Use Authorization (EUA), the Food and Drug Administration (FDA) requires that vaccine recipients or their caregivers are provided with certain vaccine-specific EUA information to help make an informed decision about vaccination. This is accomplished by providing a an EUA Fact Sheet for Recipients and Caregivers. The Fact Sheet is similar in purpose and content to vaccine information statements (VISs) for licensed vaccines but differs in that the EUA Fact Sheet is specific to each authorized COVID-19 vaccine, is developed by the manufacturer of the vaccine, and is authorized by the FDA.
